Methods used by Head Strong Foundation to collect Personal Information includes hard copy forms, online forms, surveys, registering and/or participating in Head Strong Events, requesting / purchasing merchandise, making donations, applications for volunteer or employment opportunities, signing up to Head Strong Foundation newsletter service, opting into SMS services, competitions or raffles run by Head Strong Foundation, stories submitted to Head Strong Foundation, face-to-face meetings, telephone calls, engaging in email correspondence and use of social media.
Head Strong Foundation may collect Personal Information from a Third Party. For example, Personal Information may be provided by:
- A Head Strong Foundation staff member
- A parent or guardian
- A fundraising platform for charitable purposes (E.g MyCause, GoFundMe etc)
- A treating medical professional or health professional
